Job Description

Summary

This job involves working as a social worker to help young adults and adults with mental health issues build skills for daily living, emotional stability, and community integration, mainly through referrals from a mental health center.

You'll provide hands-on support like teaching budgeting, medication management, and life skills, while coordinating care with teams and partners.

Full Job Description

The Behavioral Health area of Human Services Public Health (HSPH) is seeking two Social Workers to join the Youth Justice Behavioral Health (YJBH) team to provide Adult Rehabilitative Mental Health Services (ARMHS) to young adults and adults who are primarily referred through the Mental Health Center, which is a Certified Community Based Mental Health Clinic (CCBHC).

The ARMHS Social Workers will provide mental health services and skills training that are rehabilitative and enable the young adult or adult to develop and enhance psychiatric stability, social competencies, personal and emotional adjustment, and independent living and community skills.

Social Workers in these roles can expect to interact with the Mental Health Center, Hennepin County's Long-Term Services and Supports (LTSS) area, and other Hennepin County stakeholders.

Behavioral Health is an innovative and growing service area within Hennepin County’s Human Services department. We provide a wide range of mental health and substance use services to residents, including children, adults, and individuals involved in the criminal justice system. Our team includes over 300 staff to include: social workers, psychologists, program managers, supervisors, planners, and others—working collaboratively to achieve system goals of expanded access, upstream support, and person-centered care.

Location and hours:
These positions are hybrid and will be performed primarily in the community but also on-site at the Mental Health Center at 2215 Lake Street, Minneapolis, MN 55407, and remote as job duties require. Typical work hours will be Monday through Friday, 8 a.m. to 4:30 p.m., and occasional evening hours will be required. While these positions are designated as hybrid, based on current requirements hires must reside in or relocate to Minnesota or Wisconsin.

In these positions, you will:
  • Provide comprehensive behavioral health services and direct skill services to young adults and adults with mental illness that are referred by the Mental Health Center.
  • Instruct, assist, and support individuals in areas such as medication education and monitoring, and basic social and living skills in mental illness symptom management, household management, and employment-related or community living.
  • Assist with budgeting and money management, help with medication reminders, support the maintaining of housing, and teach daily living skills.
  • Meet with individuals and team members to determine needs and strengths, identify goals, address crises/concerns, and to develop person-centered plans to support the individual's rehabilitation and successful integration in the community.
  • Complete required documentation such as person-centered case notes, Individual Treatment Plans (ITPs), support plans, functional assessments, and track progress.
  • Provide care coordination by building partnerships with individuals served to identify and advocate for their needs while helping to reduce any barriers or racial disparities while accessing services.
  • Work collaboratively with internal and external partners and providers to facilitate the goals and services in the individual's best interest.

Need to have:
  • One of the following:
    • Bachelor's degree or higher in social work, psychology, sociology, human services, or an equivalent field.
    • Licensure as a social worker by the Minnesota Board of Social Work.
  • A valid driver's license, the ability to obtain a Hennepin County driver's permit, and daily access to reliable transportation sufficient to travel to a variety of Hennepin County community locations, to visit clients in residential settings, and to occasionally transport clients.

Nice to have:
  • Experience:
    • Working with individuals with developmental and/or intellectual disabilities, physical disabilities, mental health needs, substance use disorder, and/or medical conditions.
    • Teaching basic living and social skills.
    • Using the Social Service Information System (SSIS), Electronic Case Files (ECF), and/or the Epic electronic health records system.
    • Working with Microsoft Office (Microsoft Word, PowerPoint, Excel, OneNote, and Outlook).
  • Knowledge of:
    • Adult Rehabilitative Mental Health Services (ARMHS).
    • County and community resources for mental health, substance abuse disorder, social services, and basic needs.
    • Social work best practices and ethics.
    • Issues related to equity and ability to demonstrate a commitment to promote diversity and inclusion.
  • Strong interpersonal, organizational, computer, and person-centered interviewing skills.
