This position is located at the SC Department of Behavioral Health and Developmental Disabilities (DBHDD) - Office of Mental Health, G. Werber Bryan Psychiatric Hospital- Forensics, 220 Faison Drive, Columbia SC 29203.


Under limited supervision, you will perform diversified social work duties for a complex population consisting of clients with multiple diagnoses and psycho-social problems including criminal charges. These duties include maintaining a caseload of patient's requiring intensive psychiatric treatment by completing all documentation and clinical services related to the caseload assigned. Participate in discharge planning activities. Perform case coordination for multi-disciplinary treatment team process. You are responsible for preparation of thorough and accurate Psychosocial Assessments and Forensic Review Board Reports for assigned patients recommended for discharge. As directed, supports Forensic Outpatient Social Work initiatives.

Responsibilities for the Social Worker include:
- Maintain a caseload of patient's requiring intensive psychiatric treatment by completing all documentation and clinical services related to the caseload assigned.

- Participate in discharge planning activities and assumes the major role for implementation of referrals, scheduling of appointments and the sharing of recommendations with community agencies and programs to ensure continuity of care.


- Perform case coordination for multi-disciplinary treatment team process. Coordinate with treatment team, family, patients and other collateral sources, monitor patient's treatment and provide information for treatment team to ensure the patient's needs are being met.


- Responsible for preparation of thorough and accurate Psychosocial Assessments and Forensic Review Board Reports for assigned patients recommended for discharge.


- Will attend core competency trainings and all required trainings as assigned. Will attend monthly social work meetings and maintain Social Work Licensure, Will maintain knowledge and enforce policies and procedures.


- After at least one year of experience with the agency - you will be able to serve as field instructor or preceptor for students completing their field placement at Bryan Psychiatric Hospital.

Requirements

  • A bachelor's degree in Social Work or Social Welfare program accredited by the Council of Social Work education or from a Social Work or Social Welfare Program offered by a regionally accredited institution, plus experience as a professional social worker.

Qualifications

  • Bilingual abilities in English and Spanish (or another language) are a plus.

  • Be capable of delivering social work services to a diverse and highly vulnerable population that presents with multiple psycho-social problems with minimal supervision of the technical aspects of social work.
  • Have excellent professional judgement and be willing to participate in training.
  • Have thorough knowledge of social work principles, practices, and techniques as well as general knowledge of Federal & State laws pertaining to social service programs and CST/CR issues.
