This role involves supporting a team that helps adults and children with disabilities or health issues by handling administrative tasks, organizing records, conducting interviews, and preparing reports for community-based services.
It's a supportive position in social work that requires attention to detail and good judgment.
A good fit would be someone organized and empathetic with experience in helping vulnerable populations, who is comfortable with paperwork and occasional travel.
The Iowa Department of Health and Human Services (HHS), Division of Aging and Disability Services , Bureau of Community Integration, Targeted Case Management is seeking a Social Work Associate to join our team!
This position will be based in Des Moines, Iowa and will perform key duties to support a team of case managers and a supervisor who provide case management services to adults and children receiving in-home and community based services through various HCBS waivers including: Intellectual Disability (ID), Developmental Disability (DD), Brain Injury (BI), Health and Disability (HD), Physical Disability (PD) Habilitation (HAB), Acquired Immune Deficiency Syndrome (AIDS), and Elderly.
Hours and Travel:
Monday-Friday between 8:00am to 4:30pm. Some travel is required.
Special Requirements:
A valid driver’s license and a reliable vehicle are required for this position.
