This job involves working as a social worker at a center that supports people with intellectual disabilities, where you'll create and manage care plans, team up with others to help residents build life skills, and ensure they get the right services for a safe and healthy environment.

As a Social Worker 2, you will develop, implement, and coordinate social service programs for an assigned caseload.
You will collaborate with the Interdisciplinary Team during pre-placement planning and discharge processing, review voluntary and involuntary case intake and documentation, and ensure services provided meet the needs of individuals, families, and medical assistance standards.
You will also develop comprehensive individual care, treatment, and support services plans.
This role requires a genuine desire to foster secure and therapeutic environments that promote the development of skills needed to enjoy everyday life in the community.
In addition, you will serve as a member of the Human Rights Committee and motivate individuals to embrace their potential for improving social, economic, emotional, physical, and mental health well-being.
