The GS-13 Acute Transitions of Care Programs Social Work Supervisor/Program Coordinator provides administrative, clinical and supervisory oversight to the major specialty section, Acute Transitions of Care, including acute care, critical care, surgery care and emergency room areas at the Richard L.
Roudebush VA Medical Center.
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE).
Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited.
A doctoral degree in social work may not be substituted for the master's degree in social work.
Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the 4 accreditation standards for a masters of social work.
Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level.
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Grade Determinations: GS 13 Social Work Program Coordinator Preferred Experience: Inpatient Medicine Social Work experience and supervisory experience Experience/Education: One year of experience equivalent to the GS-12 grade level.
Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency.
Candidates may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.
Licensure/Certification: Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure.
Demonstrated Knowledge, Skills, and Abilities: In addition to the experience above candidates must demonstrate all of the following KSAs Skill in coordinating and implementing one or more specialty program(s), across the facility.
Skill in local policy and practice development, procedures, and/or practice guidelines for the program as prescribed by the program handbook or national directive.
Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).
Ability to collaborate with internal and external partners to further program goals and enhance patient centered care.
Ability to provide the full range of supervisory duties including assignment of work to be performed; performance evaluations; and selection of staff, training, and recommendation of awards, advancements, and when appropriate, disciplinary action.
Ability to oversee the fiscal matters of the functions supervised (including fund controls, contracts, and equipment expenditures), forecast resource and equipment needs, and administer the allocated budget.

The full performance level of this vacancy is 13. The actual grade at which an applicant may be selected for this vacancy is a 13.
Physical Requirements: The physical demands of the position involve standard office requirements such as using the telephone, retrieving files, writing reports, data entry and maintenance of computer databases, etc.
Work is typically performed sitting at a desk and using a computer.
This position may require occasional travel and therefore the incumbent must have a valid drivers license and be able to operate a government vehicle.
The position requires periods of walking, standing, sitting and bending. Major Duties:
Duties included but not limited to: Supervises a group of employees performing work at various GS levels up to their own grade and provides administrative, technical, and clinical supervision necessary for accomplishing the work of the programs.
Establishes guidelines and performance expectations for staff that are clearly communicated through the formal employee performance management system.
Observes workers' performance and conducts work performance critiques, providing informal and formal feedback through supervision. Periodically evaluates employee performance by VA policy/procedure.
Provides advice and counsel to workers related to work and administrative matters. Effects disciplinary measures as appropriate to the authority delegated in this area.
Resolves informal complaints and grievances. Reviews and approves or disapproves leave requests.
Exercises supervisory authorities and responsibilities involving work assignment and review, as well as the administrative and personnel management functions relative to the staff supervised.
Oversees program operations and evaluations, identifying areas for improvement, gathering relevant data, assessing the data, developing and implementing ideas for improvement and evaluating efficacy of improvement efforts.
Serves as the VHI subject matter expert regarding both discharge planning and guardianship, providing consultation for Veterans and staff of VHI.
Assists Veterans and/or support person(s) with guardianship acquisition Works with courts, law enforcement, and other applicable institutions to ensure Veteran's access to timely, comprehensive services.
Provide outreach and education to assist Veterans, legal counsel, judges, VA clinical staff and other community stakeholders.
Establishes partnerships with community agencies to help facilitate proactive planning in an effort to avoid future guardianship proceedings for those Veterans who have limited social support.
Serves as a member of the Social Work Service Leadership Team, taking an active role in implementing and supporting service-related priorities, initiatives and committee participation.
