The Colorado Mental Health Institute at Fort Logan (CMHI-FL) is one of two state hospitals serving our community. The vision of CMHIFL is to provide the highest quality mental health services to persons with complex, serious, and persistent mental illness within the resources available. Our mission is to support those individuals in achieving their recovery goals and reaching their full potential. CMHIFL provides inpatient treatment to adult patients, generally between the ages of 18 and 59. The hospital currently has 138 adult inpatient beds, over six treatment milieus serving civil and forensic individuals. Medical services, occupational therapy, neuropsychological rehabilitation services, and trauma-informed care services are available to all patients throughout the hospital.

The Social Work III position is based at the 24-hour State psychiatric hospital, working with individuals who are voluntarily and involuntarily committed for treatment. Patients receiving treatment at CMHI-FL have severe and persistent mental illness and may have traumatic brain injury, trauma disorders, and other complex dynamics. Each treatment team is multidisciplinary, including psychiatrists, psychologists, social workers, addiction specialists, occupational therapists, recreation therapists, creative arts therapists, nurses, physicians, and mental health clinicians. In addition, the hospital offers dietary, peer, and chaplain services

The Social Work unit collaborates with patients, their families and/or support systems, and external stakeholders to plan, organize, and execute psychiatric treatment interventions and comprehensive aftercare plans with placement. Services include clinical assessment, treatment planning, individual therapy, group therapy, family therapy, counseling, and education. This work unit serves as a liaison between the hospital and Community Mental Health Centers (CMHC’s) and other governmental entities.
The purpose of the Social Worker is to provide individual therapy, group therapy, and family therapy. Individual and family therapy are provided as needed, which is determined in the treatment plan meetings and in collaboration with the treatment team. Group therapy is offered to all patients and group assignment shall be determined by clinical and individualized need. The Social Work positions are responsible for developing group materials appropriate for patient specific needs and leading group therapy sessions. The Social Worker completes comprehensive biopsychosocial assessments and reassessments, coordinates treatment planning for all patients, and regularly documents in the electronic health record. The Social work team is responsible for coordination of discharge and comprehensive aftercare plans. In doing so, this position collaborates with support systems, stakeholders, CMHC’s, and other agencies to coordinate continuity of care. The Social Worker actively contributes to hospital-wide events, in-service training, and program development.
The purpose of the Social Worker III "Float" position is to ensure continuity of clinical and social work services across the Civil and Forensic Units of the Colorado Mental Health Hospital in Fort Logan (CMHHi-FL) by providing essential coverage, training, and administrative support to the Social Work Department. This position is responsible for facilitating scheduled therapeutic groups, temporarily managing caseloads and providing group therapy coverage during staff leave or urgent departmental needs. Additionally, the role is integral to professional development by offering supervision toward licensure and facilitating clinical training for new employees at New Employee Orientation (NEO) and beyond. This position also provides necessary administrative support to enhance the efficiency of discharge planning and other operational processes. Other duties include:


Clinical Coverage and Direct Service:
  • Provides essential social work and clinical coverage across both Civil and Forensic Units, ensuring continuity of patient care during staff absences, periods of high census, or other urgent departmental requirements. Responsibilities include:
  • Providing Social Work case coverage and temporarily managing a caseload for personnel on extended leave or during periods of vacation.
  • Delivering group coverage to clinical staff during instances of illness, vacation, or urgent departmental requirements.
  • Facilitating 3-4 scheduled clinical groups per week to meet patient treatment needs.
  • Supporting the completion of admission assessments when a team social worker is unavailable.
  • Maintains autonomous clinical standards, ensuring all therapeutic interventions, assessments (including biopsychosocial assessments), treatment planning coordination, and documentation in the electronic health record adhere to hospital policy, regulatory requirements, and best practices.

Training and Professional Development:
  • Contributes to the professional development and orientation of the Social Work department and new hospital staff. This includes:
  • Offering professional supervision toward licensure for social work staff and trainees.
  • Facilitates a weekly Group Supervision for the social work department
  • Instructs new employees during New Employee Orientation (NEO)
Administrative and Discharge Support:
  • Coordinates transportation logistics for discharge planning appointments and transfers.
  • Assists with the preparation and submission of referral packets for community step-down programs to support discharge planning
Other Duties as Assigned:
  • Other duties as assigned include hospital-wide ad hoc activities, milieu support, etc.

Qualifications

Education/Licensure/Certification/Experience:

Current, valid Colorado licensure as a:

  • Licensed Social Worker (LSW) OR

  • Licensed Clinical Social Worker (LCSW) OR

  • Licensed Professional Counselor (LPC) OR

  • Licensed Marriage and Family Therapist (LMFT)

  • Licensed Addiction Counselor (LAC)

And one (1) year of licensed experience based on the work assigned to the position.


Preferred Qualifications:
  • Social Work Degree
  • SPMI experience
  • Previous State experience
Conditions of Employment:
  • May be exposed to bodily fluids or caustic chemicals
  • May travel to attend training, court hearing, escort patients to appointments, and other related activities
  • TB testing, annual influenza, and other vaccines as required by the State and facilities
  • Current, valid licensure
  • Due to the juvenile population served at the MHI facilities, incumbents of this position must be at least 21 years of age.

CDHS Selection Process Explanation
