The Riverside University Health System - Behavioral Health is currently recruiting a Registered Nurse II or Registered Nurse III to join their Justice-Involved Enhanced Care Management (ECM) team located in Indio. The ECM program serves individuals with complex medical, behavioral health and social needs, including those transitioning from incarceration. Our program utilizes a whole-person care approach, with a strong focus on helping members obtain and maintain stable housing.


This is a field-based Registered Nurse position that requires travel throughout Riverside County to meet members in community settings including correctional facilities, hospitals, clinics, and housing program locations. A valid CA Driver's License is required. Bilingual is preferred but not required.

The selected Registered Nurse will conduct comprehensive nursing assessments and contribute to the development of individualized care plans for members. The RN will provide care coordination, case management, and health education for members with complex needs.


In collaboration with an interdisciplinary team-including Behavioral Health Specialists, Peer Support Specialists, Community Services Assistants, and Housing Navigators the RN will support members in navigating medical, behavioral health, and community-based services. The RN will also coordinate care transitions from correctional facilities, hospitals, and other institutions to ensure continuity of care.

Additionally, the RN will support housing stabilization efforts by coordinating with housing providers and community partners. Responsibilities include documenting services and care coordination activities in required electronic health record systems. The RN will also provide clinical oversight for ECM members, including reviewing and signing off on assessments and clinical documentation completed by care team staff as well as offering clinical consultation to the care team on complex member cases.

In addition to the duties above, the Registered Nurse III may facilitate interdisciplinary case conferences or team meetings, support workflow improvement, staff mentorship, and quality improvement efforts and perform additional program-related duties within scope of practice as assigned by the supervisor

The most competitive candidates will possess experience working with underserved populations, including individuals experiencing homelessness, behavioral health conditions, substance use disorders, or those transitioning from incarceration.

All ECM team members support housing navigation practices and coordinate care with case managers and community partners involved in housing navigation and housing stability efforts.

Experience with care coordination programs, community-based healthcare delivery, housing-focused services, and electronic health record systems (Epic, Care Director, Manifest Medex, or Managed Care Plan Provider Portals) is desirable.


This is a full-time position that requires the following licenses and certifications:
  • A current and valid license to practice as a Registered Nurse in the State of California
  • A current and valid Basic Life Support, (BLS) certification, issued by the American Heart Association.
  • A valid CA driver's license.

Work schedule: Monday - Thursday 7:30am-5:00pm Friday 7:30am - 4:00pm with every other Friday off.

Requirements

• Within the full scope of the Nurse Practice Act, assess, plan, organize, and provide nursing care to assigned patients in accordance with physician instructions; make preliminary observations of, and prepare patients for, medical treatment.
• Assist physicians in administering treatments and in performing medical examinations, diagnostic tests, and surgical operations.
• As directed, administer prescribed treatment and medications including dispensing as ordered to patients; chart treatment of patients and record and report significant changes in condition and general progress of patient.
• Counsel and advise patients, families and significant others regarding special medical problems and/or proper healthcare methods; prepare and maintain concise and complete records and reports
• Instruct patients in carrying out physician's orders; may transcribe physician's orders to working records.
• May provide orientation and training to new staff, residents, medical students, ROP and nursing students.
• May participate in quality assurance reviews; may act as a team leader or relief charge nurse.
• Assign and monitor the work of subordinate nursing personnel; participate in committee activities.
• May triage patients by reviewing admission charts and merging patients into the patient assignment or clinic flow at the appropriate time and place.
• May testify in court as to the client's mental and physical condition; may recommend initiating conservatorship investigations when assigned to the mental health setting.
• May coordinate and monitor patients through the hospitalization process utilizing physicians, primary care nurses, and third-party payors to maintain quality care and fiscal responsibility.
• May review the post hospital care plan with the patient/family, establishing a contact regarding time frames and responsibilities; follow plan through a discharge.

Qualifications

Registered Nurse II

Education: Successful completion of the education required to obtain a valid license to practice as a Registered Nurse in the State of California.

(A bachelor's degree from an accredited college or university in nursing or a health-related field may be required for some positions.) Experience: Completion of one year as a Registered Nurse in an acute care hospital, mental health setting, clinic outpatient setting, or healthcare agency program.

Experience gained as an Interim Permit Nurse may be credited toward meeting the required experience. Evaluations and references must show proficiency of practice as a Registered Nurse.

Registered Nurse III

Education: Successful completion of the education required to obtain a valid license to practice as a Registered Nurse in the State of California.

Experience: Three years of full-time experience as a Registered Nurse in an acute care facility, mental health facility, or community health agency with references and evaluations to show appropriate skill level.

(A bachelor's degree from an accredited college or university in nursing or national certification in a nursing specialty may be considered as a substitution for one year of the required experience.

A master's degree from an accredited college or university in nursing may be considered as a substitution for two years of the required experience.)

Other Requirements

License/Certificate: Must possess and maintain a current valid license to practice as a Registered Nurse in the State of California.

Possession of valid Basic Life Support (BLS) certificates issued by the American Heart Association.

Possession of a valid California Driver's License may be required.

Knowledge of: Current principles, techniques and procedures used in professional nursing; medical terminology and patient care routine appropriate to clinical setting; medical instruments and equipment; the uses and effects of medicines and narcotics; community resources available to provide patient care and follow-up; federal, state and local laws and regulations governing professional aspects of nursing; identification of potential discharge needs; payor source documentation requirements and governmental regulations affecting reimbursement.

Ability to: Perform the full range of nursing duties and responsibilities in a patient care setting; plan and organize nursing plans for patients; prepare and maintain concise and complete records and reports; establish and maintain effective working relationships with physicians, patients, and fellow employees; assist in the instruction of entry-level nurses and ancillary personnel; evaluate the medical records of hospital admissions; make recommendations regarding continued stay necessity; identify discharge needs.
