In this position, you will coordinate and oversee the collaborative, interdisciplinary assessment, and care planning process for skilled care residents.
Your work will involve evaluating these processes to ensure compliance with regulations.
You will assist with the management of clinical report meetings, monitor care plan documentation, and interview residents as prescribed by state and federal regulations.
Some of your duties will include initiating a baseline care plan and attending interdisciplinary team meetings to gather information, communicate changes, and update records.
Additionally, you will participate in clinical care rounds, manage projects, and investigate incidents to support resident care and safety needs.
