Summary

This role involves working as a public health nurse to assess health needs, create personalized care plans, and provide support to people in their homes and communities, focusing on behavioral health services.

Key Requirements

  • Bachelor's degree in nursing from an accredited college or university, OR diploma in nursing plus one year of professional nursing experience, OR associate's degree in nursing plus two years of professional nursing experience
  • Valid Registered Nurse (RN) license in Maryland

  • Experience in case management, monitoring, and evaluation of care plans
  • Knowledge of state, federal, and Medicaid regulations for compliance
  • Skills in health screening, education, counseling on disease prevention, health maintenance, and mental health

Requirements

Job Purpose
Under general supervision, plans, coordinates, and provides professional public health nursing assessment and person-centered care planning to individuals, families, and groups in home and community-based settings.

Essential Job Duties

Provides nursing assessments, person-centered care planning and referral services to individuals, families, groups, and institutions.Review Maryland Department of Health Long Term Services and Supports Tracking System (LTSS) assessment requests upon assignment of cases.Contact assigned clients/representatives timely.

Conducts home visits.Complete nursing assessments and high-quality person-centered care planning timely to ensure timely access to home and community-based services.Provides case management, ongoing monitoring and evaluation of person-centered care plan.Assesses and monitors clients' physical and mental states of health and determines need for community-based resources.Ensures compliance with state, federal and Medicaid regulations.Provides health screening education and counseling in the areas of disease prevention, health maintenance, and mental health.Participates in the instruction and orientation of new public health nurses.Prepares and maintains nursing records and reports.

Qualifications Required QualificationsGraduation from an accredited college or university with a bachelor's degree in nursing.OR Graduation from an accredited diploma school of nursing:PlusAt least one-year of professional nursing experience.Additional qualifying education may be substituted on a year-for-year basis for the required experience.ORGraduation from an accredited college or university with an associate in arts degree in nursing;Plus At least two years' professional nursing experience.Additional qualifying education may be substituted on a year-for-year basis for the required experience.Licenses and CertificatesPossession of a valid license to practice as a professional registered nurse, issued by the Maryland State Board of Examiners of Nurses, is required at the time of appointment.

Possession of a valid driver's license equivalent to a noncommercial, class C Maryland driver's license is required at the time of appointment.Proof of Licenses, Certifications and EducationApplicants are required to submit proof of education beyond high school to meet the required and preferred qualifications of the position.

Diplomas or transcripts must show the applicant's major field of study. Copies and unofficial transcripts are acceptable.
