The Program Support Assistant serves as the Patient Centered Management Module Coordinator (PCMMC) for the Miami VA Healthcare System.
The Program Support Assistant works in collaboration with the Assistant Chief Of Staff (ACOS and the Administrative Officer for Ambulatory Care Service, Providers and other clinical staff in ensuring the management and accuracy of the provider panel data base.
To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 04/23/2026.
Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement.
For a GS-07 position you must have served 52 weeks at the GS-06. The grade may have been in any occupation, but must have been held in the Federal service.
An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials.
If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement.
In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade.

Examples of specialized experience would typically include, but are not limited to: communicating with internal and external customers of the organization, preparing correspondence, meeting agendas, meeting minutes, and follow-up action items with appropriate staff members, conducting surveys, maintaining data, reports, and publications, assists team with data collection maintenance and monthly report, coordinating hospice related public outreach efforts, appointment scheduling, room assignment, and follow-up.

Major Duties:
Incumbent is the Primary Care Management Module Coordinator (PCMM) for Primary Care Teams within the Health Care System (HCS) service area.
He/she works in collaboration with the team leaders, Clinical Managers, Providers, Nurses, Clerks and the rest of the clinic staff, CBOC staff and contract providers and is responsible for maintaining the integrity of the PCMM database for all of Primary Care throughout the hospital, outpatient clinics and CBOC's service area.
The PCMM will set up and define teams; assign staff to positions on defined teams; assign patients to teams, assign patients to practitioner via team position and enroll in clinic; complete reports for health care team; manage inactivation list to include but not limited deaths, potential inactivated patients, transfers, and dual assignments.

Enters Primary Care information into PCMM system, enrolls patients into the Primary Care teams, and assigns patients to the appropriate provider within the PCMM system.
Incumbent works with Team Leaders and Clinic Managers to formulate and conduct training and orientation for all providers and clerks involved with PCMM.
Provides expertise and advice to all providers involved with PCMM. Manages administrative functions associated with the Primary Care PCMM program for the facility and clinics.
Assists professional staff with and handles administrative functions of the office. Gathers administrative information regarding patients for clinic managers.
Receives all calls for the Caregiver Support Program at the Miami Facility, address's each call and or routes to the appropriate staff or area. Work Schedule: Monday through Friday, 8:00 am to 4:30 pm.
