This job involves managing the Patient Centered Management Module (PCMM) system to support patient care teams in a VA hospital, including working with doctors, nurses, and other staff to keep patient data accurate and improve clinic operations.

Key Requirements

Full Job Description

The Patient Centered Management Module (PCMM) Program Specialist for Ambulatory Care/Primary Care at the Orlando VAHCS and collaborates with Ambulatory Care Leadership, Primary Care Section Chiefs, Nurse Managers, Primary Care Physicians, HAS, Pharmacy, Nutrition, Social Work, Mental Health, Whole Health, Home Based Primary Care, Spinal Cord Injury, Infectious Disease, Transition Care, GPM, Chief of Staff, and Director's Office, and staff to maintain program integrity and data.

To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 03/20/2026.

Preferred Experience: 1 full year of graduate level education Experience with PCMM software (highly preferred) Experience in Primary Care/Patient Aligned Care Team setting Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement.

GS-7: You must have at least 52 weeks service in position no more than two grades lower (GS-05) GS-9: You must have at least 52 weeks service in position no more than two grades lower (GS-07) The grade may have been in any occupation, but must have been held in the Federal service.

An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials.

If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement.

In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade.

Examples of specialized experience would typically include, but are not limited to: Apply knowledge of VHA & Orlando VA Health Care System policies, procedures, and VHA directives and regulations; Adapt analytical techniques and evaluation criteria for program measurement and improvement; Leverage extensive knowledge of primary care operations and systems; Develop and modify work methods, management processes, and administrative procedures; Incorporate in-depth knowledge of clinical practices related to patient management, consults, clinic creations, and maintenance; Understand and implement Patient Aligned Care Teams (PACT), Behavioral Health services, Non-Primary Care Services, and Community Care Services; Operate outpatient processing systems, VISTA Scheduling package, and appointment management software; Gather, analyze, and evaluate clinic process information; Utilize Pyramid Analytics, CPRS Electronic Health Record, VISTA, and PC software programs (Word, Excel, Power Pivot, PowerPoint) to compile and extract data and create reports; Plan, set priorities, and coordinate activities; Execute various analytical tasks to conduct comparative studies.

Major Duties:

Duties and responsibilities include, but are not limited to: Maintain application functionality, utilization, and resulting data. Serve as technical expert and troubleshoot user issues.

Regularly meet with Primary Care leadership to review provider panel management data and staffing ratios. Delegate PCMM user permissions to facility staff.

Establish and manage Patient Aligned Care Team (PACT) team-lets, position setup, staff, their FTE assignments, and patient assignments. Maintain and develop streamlined processes affecting PCMM.

Maintain oversight of Multi-PACT approval and disposition for timely completion of active assignments. Setup and define teams, assign staff positions, and assign patients.

Allocate user permissions and manage reports, outputs, and mail messages. Manage the hospital's large PCMM database. Coordinate with various departments to maintain data integrity.

Resolve alerts and disseminate PCMM-related information to stakeholders. Oversight on assignment and attrition based on patient data. Work with PACT staff for clinical input on patient panel attrition.

Determine and coordinate Veteran patient panel assignments. Comply with VHA Handbook policies for exceptions and multiple assignments. Collect, compile, and track data in support of program operations.

Perform database searches, prepare reports, and maintain records. Monitor and validate data related to PCMM. Conduct internal audits and oversee patient processing activities.

Assist in the preparation of statistical reports for Ambulatory Care Service.

Work Schedule: Monday-Friday, 8:00am-4:30pm Travel-Periodic Basis: Travel to facility CBOCs and divisions/sites of care is required to verify/validate rooms, staffing, and primary care setup.
