The VISN Clinical Resource Hub (CRH)Primary Care position is a VISN based position located in VISN 5.
A Clinical Resource Hub (CRH) is a repository of clinical and administrative staff that serve facilities within a Veterans Integrated System Network (VISN).
Primary Care Services are provided to VISN medical centers and Community Based Outreach Clinics (CBOC).
To qualify for this position, you must meet the basic requirements as well as any additional requirements (if applicable) listed in the job announcement.
Applicants pending the completion of training or license requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Currently employed physician(s) in VA who met the requirements for appointment under the previous qualification standard at the time of their initial appointment are deemed to have met the basic requirements of the occupation.
Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Degree of doctor of medicine or an equivalent degree resulting from a course of education in medicine or osteopathic medicine.
The degree must have been obtained from one of the schools approved by the Department of Veterans Affairs for the year in which the course of study was completed.
Current, full and unrestricted license to practice medicine or surgery in a State, Territory, or Commonwealth of the United States, or in the District of Columbia.
Residency Training: Physicians must have completed residency training, approved by the Secretary of Veterans Affairs in an accredited core specialty training program leading to eligibility for board certification.
(NOTE: VA physicians involved in academic training programs may be required to be board certified for faculty status.) Approved residencies are: (1) Those approved by the Accreditation Council for Graduate Medical Education (ACGME), b) OR (2) Those approved by the American Osteopathic Association (AOA),OR (3) Other residencies (non-US residency training programs followed by a minimum of five years of verified practice in the United States), which the local Medical Staff Executive Committee deems to have provided the applicant with appropriate professional training and believes has exposed the physician to an appropriate range of patient care experiences.
Residents currently enrolled in ACGME/AOA accredited residency training programs and who would otherwise meet the basic requirements for appointment are eligible to be appointed as "Physician Resident Providers" (PRPs).
PRPs must be fully licensed physicians (i.e., not a training license) and may only be appointed on an intermittent or fee-basis.
PRPs are not considered independent practitioners and will not be privileged; rather, they are to have a "scope of practice" that allows them to perform certain restricted duties under supervision.
Additionally, surgery residents in gap years may also be appointed as PRPs. Proficiency in spoken and written English.
Preferred Experience: Board Certified or Board eligible in Family or Internal medicine and Telehealth experience.

Physical Requirements: The work is mostly sedentary and requires some standing, bending and carrying light items.
The incumbent must have the ability to stand and walk for prolonged periods of time to interact with patients and other services. Major Duties:
This position is in the VISN 5 Primary Care Clinical Resource Hub (CRH).
The position utilizes a clinical video telehealth (CVT), VA Video Connect (VVC), and telephone modalities to deliver primary care services to patients in other clinics throughout VISN 5 in addition to quarterly on-site care delivery.
The major duties of the position include: - Perform initial histories and physical examinations of outpatients.
History and physical examinations will mostly rely on the use of CVT and VVC technology, but some direct face-to-face visits may also be needed. - Provide ongoing outpatient care for assigned patients.
- Screen outpatients to determine need for further care. - Prescribe medications within defined scope of practice.
- Order diagnostic studies such as laboratory tests, x-rays, electrocardiograms (EKG's). - Review need for and order health promotion and disease prevention activities.
- Initiate, respond to, and expedite requests for consultations, special tests and studies.
Provide education and counseling of patients and families in preventive care, medical conditions, and use of prescribed treatment and medications.
There are back-up services available in the local and VA emergency rooms (near the remote clinic site) as well as surgery, psychiatry, and medical specialties (affiliated with the remote clinic site.) Patients who need admission to the inpatient wards will be transported to the nearest VA facility when appropriate.
The physician may sometimes need to work alone but can request assistance from other medical staff (including other telehealth primary care staff, as well as staff in the Emergency Room or inpatient service at the nearest VA Medical center when needed.) Shifts may occasionally deviate from the usual 8:00 a.m.
to 4:30 p.m. shift schedule. The facility is fully computerized, and computer skills are required. Special training will be provided to utilize CVT technology.
The physician will provide direct patient care full time and work as a member of an expanded healthcare team (social worker, dietician, clinical pharmacist, behavioral health clinician, and a care manager) to oversee and coordinate medical care for a population of Veterans in a group practice setting utilizing the Patient Aligned Care Team model.
HAS (Health Administration Service) and Nursing staff will usually be provided by the remote clinic location/facility.
The practice emphasizes evidence-based medicine, primary and secondary preventative medicine, the use of computerized medical record system and continuous quality improvement based on individualized provider data.

Education Debt Reduction Program (Student Loan Repayment):Authorized: This position is eligible for EDRP, a student loan payment reimbursement program.
You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of your start date.
Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office.
