Full Job Description

Hennepin Health (HH) is looking for a Community Based Nursing Supervisor to join its Social Service Navigation (SSN) team.

HH provides health care coverage to Hennepin County residents who are enrolled in a Minnesota Health Care Program. Our mission is to help people improve their health through innovative collaborations.

In this position, you will direct, assign, and review the work of the SSN team which will include supervisory functions such as performance reviews, hiring, team coordination, discipline, and administrative duties related to policies and procedures, and compliance and HR, in addition to developing annual team goals, care coordination performance standards, and objectives in collaboration with the area manager.

This position will contribute to the preparation of the annual Quality Management and Population Health plans and compile analytical reports and respond to data requests from multiple sources as well as develop and train staff on resource navigation that will include culturally responsive and expansive options.

This position will be supervising an interdisciplinary team of professionals including licensed RNs, social workers and certified substance use disorder (SUD) specialists who are performing care coordination work in the field and community-based settings.

Location and hours:
This position is hybrid and will be performed both on-site at Hennepin Healthcare (HCMC Hospital) at 730 South 8th Street, Minneapolis, MN 55404, the Health Services Building at 525 Portland Avenue, Minneapolis, MN 55415, and remote as job duties require. Typical work hours will be Monday through Friday between 8 a.m. - 5:30 p.m. While this position is designated as hybrid, based on current requirements hires must reside in or relocate to Minnesota or Wisconsin. New Employee Orientation (NEO) for all new benefit-earning employees is in-person in downtown Minneapolis. Details about NEO will be provided upon hire.

In this position, you will:
  • Participate in integrated, multidisciplinary, or community initiatives.
  • Serve as a liaison between Hennepin County and communities, community organizations, schools, health professionals, and service providers in collaboration with the area manager.
  • Identify needs and assist in resolving problems with individuals and families regarding health and human service issues (including but not limited to housing, employment, financial assistance, and behavioral health).
  • Review, assign, and direct the work of the Hennepin Health SSN interdisciplinary team. Perform supervisory functions including evaluating performance, discipline, hiring, and terminating staff.
  • Provide team coordination, ensure compliance with the DHS contract, National Committee for Quality Assurance (NCQA) requirements, internal departmental and Hennepin County policies, and Accountable Care Organization partner agreements.
  • Collect and maintain program data and reports.
  • Contribute to contractually required reports, compile analytical reports, respond to data requests from multiple sources, maintain records, and monitor and track team metrics and performance indicators.
  • Develop annual team goals and care coordination standards and objectives in collaboration with the area manager. Execute and enforce policies, procedures, and fulfillment of DHS contractual requirements.
  • Resource navigation development and training. Includes culturally responsive and expansive options for the team along with keeping this updated regularly.

Need to have:

  • Bachelor's degree or higher from an approved school of nursing and two or more years of professional nursing experience.
  • Licensure as a Registered Nurse issued by the state of Minnesota.
  • A valid driver's license and access to reliable transportation to attend meetings and visit community sites throughout Hennepin County.

Nice to have:
  • Experience:
    • Working with adults with mental health illnesses and/or substance abuse.
    • Supervising an interdisciplinary team.
    • Participating in multidisciplinary or community initiatives to improve community health.
    • Using Epic or a similar Electronic Health Record (EHR) system.
    • Working with individuals facing homelessness.
  • Knowledge of:
    • Theories, techniques, and practices of the specialty area and current public health and harm reduction best practices and principles.
    • Educational or training methods and community resources.
    • Program development, evaluation, and research methods.
    • Community resources, educational or training methods, and the eligibility requirements for resources that support individuals.
  • Ability to:
    • Establish effective working relationships with professional and non-professional groups.
    • Exercise independent judgment and initiative in performing assigned duties.
    • Communicate both verbally and in writing to partner agencies and internal teams.
    • Successfully manage multiple priorities and projects.
    • Sit, stand, walk, and lift and carry up to 25 pounds, with or without reasonable accommodation.
