Job Description

Summary

This job involves visiting homes to provide health support and guidance to expectant mothers and their families through the Nurse Family Partnership program, helping them with physical, emotional, and social needs during pregnancy and early childhood.

It's a community-focused nursing role in public health, funded by grants, where you'll build relationships, promote the program, and manage a caseload of about 25 clients.

A good fit would be a registered nurse with a bachelor's degree who enjoys working independently in homes, has experience in maternal or child health, and is comfortable with flexible hours and community outreach.

Key Requirements

  • Bachelor of Science Degree in Nursing (BSN) or higher
  • Two (2) full-time years of related work experience in nursing
  • Current Registered Nurse (RN) license
  • Valid Driver's License and good driving record
  • Ability to conduct home visits and manage a caseload of 25 active clients
  • Completion of all required Nurse Family Partnership (NFP) training and continuing education
  • Strong communication skills and ability to work autonomously under NFP guidelines

Provide comprehensive community health nursing services to women and their families eligible for the Nurse Family Partnership (NFP), according to the guidelines of the NFP.

Requirements

1. Provides home visits for women and families in the NFP Program.
2. Supports the guidelines and standards of the NFP Program and Public Health.
3. Promotes awareness of NFP throughout Tarrant County.
4. Develops and maintains community relations to support NFP referrals.
5. Completes all required NFP training and continuing education.
6. Assesses physical, emotional, social, and environmental needs of women and their families as they relate to the NFP guidelines.
7. Consults and collaborates with other professionals involved in providing services to women and their families.
8. Maintains a caseload of 25 active clients and fulfills reporting responsibilities.
9. Performs all other related duties involved in the operation of the Public Health Department as assigned or required.

Qualifications

TO APPLY, must have a Bachelor of Science Degree in Nursing (BSN) or higher + Two (2) full-time years of related work experience.

MUST PROVIDE LICENSING AGENCY AND LICENSE NUMBER ON APPLICATION IN THE "License" FIELD OR RISK DISQUALIFICATION.

TO APPLY, must possess a valid Driver License and a good driving record.

Experience with community, maternal, child health or mental and behavioral health is preferred.

Good knowledge of fundamental concepts, practices and procedures of field of nursing.

Good communication and presentation skills.

Ability to make professional nursing decisions and operate autonomously under the NFP guidelines.

Bilingual preferred.

Flexible work availability is needed to facilitate home visits.


If hired, must provide proof of educational attainment
at New Hire processing or during the promotional process.
Tarrant County will conduct background checks on new hires that will include a criminal background check related to convictions and deferred adjudications in the past seven years and may include credit reports, motor vehicle records, employment records and educational attainment. A conviction or deferred adjudication is not an automatic bar to employment. Each case is considered individually.

Additional Information

While performing the duties of this position, the incumbent is regularly required to sit, stand, walk, bend, stoop, lift, drive, push and pull.
