Full Job Description
The Hennepin County Behavioral Health area is seeking a Nurse Practitioner for the Mental Health Center (MHC). The person hired for this role will assess and provide ongoing psychiatric treatment for children and adolescents with mood disorders, attention-deficit hyperactivity disorder (ADHD), conduct disorders, autism spectrum disorders, developmental disabilities, and other serious emotional disorders. This work will also include assessing and providing ongoing psychiatric treatment for adults with various mental health needs. This person will work closely with staff psychiatrists, psychologists, psychotherapists, nurses, and other nurse practitioners to provide comprehensive psychiatric care to Hennepin County residents.Candidates who are interested in the National Health Service Corps should know that the Mental Health Center is designated as a Health Professional Shortage Area (HPSA). Location and hours:This onsite position is located at the
Mental Health Center (MHC) in the South Hennepin County Human Service Center at 2215 East Lake Street, Minneapolis, MN 55407. Typical work hours are between 8 a.m. to 6 p.m. Monday through Friday, with limited early evening hours. No weekend, overnight or holiday shifts are required. New Employee Orientation (NEO) for all new benefit-earning employees is in-person in downtown Minneapolis. In this position you will: - Perform initial and ongoing assessments of patients' psychiatric, medical, physical, and psychosocial status.
- Provide ongoing medication management for patients, including assessing effectiveness and adjusting medications for optimum effectiveness with minimal side effects; consult with other providers and family members.
- Provide direct care, monitoring and/or referral for clients with co-occurring physical/medical conditions and co-occurring mental health and substance use disorder.
- Obtain consultation with other prescribers and/or primary care physician.
- Provide psychiatric consultation and/or care coordination to other providers.
- Document all interactions and findings in electronic health records in a timely and accurate manner.
- Function as part of a multidisciplinary team in support of the MHC goals and mission.
- Participate in quality improvement efforts aimed at equity and disparity reduction.
Need to have:- One of the following:
- Possession of a valid license as a Registered Nurse issued by the State of Minnesota and National Certification as a Nurse Practitioner (CNP).
- Possession of a valid license as an Advanced Practice Registered Nurse issued by the State of Minnesota.
- Certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP).

- Establish collaborative working relationships with clients, families, and other service providers.
- Demonstrate sensitivity to the needs and issues of diverse and disenfranchised persons and families.
- Knowledge of:
- Trauma-informed care and other clinical best practices.
- Child protection and juvenile justice systems.

The Mental Health Center (MHC) is a state-licensed outpatient clinic. The MHC’s team of support and provider professionals provides comprehensive outpatient behavioral health services to adults with serious mental illness as well as those with co-occurring substance use disorders. Additionally, the Center services children, youth, and families, many of whom are involved with Child Protection or Juvenile Justice. With state Certified Community Behavioral Health Clinic (CCBHC) certification pending, we’ve advanced our goals of service excellence and the provision of personalized, coordinated, equitable care for persons we serve.
