Hennepin County Mental Health Center (MHC) is looking to hire a skilled Licensed Practical Nurse to support Hennepin County residents in achieving their health goals and recovery through access to low-barrier and high-quality health interventions provided at both of our Minneapolis locations.

Location and hours:

This position is located at the Mental Health Centers at the South Minneapolis Service Center at 2215 East Lake Street, Minneapolis 55407, and at the Behavioral Health Center at 1800 Chicago Avenue, Minneapolis, MN 55404.

Work hours vary depending on location and start as early as 8 a.m. and end as late as 6 p.m., Monday through Friday. Occasional evening availability may be required.

New Employee Orientation (NEO) for all new benefit-earning employees is in-person in downtown Minneapolis. Details about NEO will be provided upon hire.

In this position, you will:
  • Provide practical nursing care to patients and their families.
  • Collect and validate physical and psychosocial data, including vital signs.
  • Contribute to the initial and ongoing assessment of patient care needs and the development of patient care, teaching, and discharge plans.
  • Observe and monitor patients, reporting deviations from normal, response to treatment, and changes in behavior.
  • Communicate with members of the health care team.
  • Administer prescribed medications, including the management of medication refills and prescribed treatments, such as basic wound care.
  • Assist in physical examinations and treatments as needed, including the management of IV therapy, activities of daily living, and emergencies.
  • Provide teaching to meet the needs of patients and families, according to teaching plans.
  • Review written discharge instructions with patients and families and document on appropriate forms and complete discharge forms.
  • Perform care coordination, including setting up appointments and transportation to medical facilities and appointments outside of Hennepin County.
  • Monitor performance of Licensed Practical Nurse, Nursing Assistant, Health Unit Coordinator, and Health Care Aise as assigned.
  • Under general supervision, work as the nurse in charge when assigned by a supervisor.
Need to have:
  • Completion of an approved program in practical nursing and successful completion of the State of Minnesota's (or equivalent) nursing exam.
  • Possession of a license as a Licensed Practical Nurse (LPN) authorized by the State of Minnesota.
Nice to have:
  • One year or more experience providing professional nursing in behavioral health or integrated care setting.
  • Current CPR certification.
  • Experience:
    • Administering medications via intramuscular route.
    • Working with laboratory management, point-of-care testing, or phlebotomy.
    • Using electronic health records to schedule and register clients and document client data/care.
    • Working with:
      • Underserved populations.
      • Adults experiencing homelessness and mental illness or co-occurring psychiatric and substance use disorders.
      • Or in a primary care clinic, home care, or performing care coordination duties.
  • Knowledge of:
    • Mental health diagnosis and medications
    • The Substance Abuse and Mental Health Services (SAMHSA) 10 x 10 Wellness Campaign
