CO State Veterans Community Living Center at Rifle

The Colorado State Veterans Community Living Center at Rifle is located on Colorado's western slope in the scenic Colorado River valley. Rifle offers a mild climate, with plenty of clean mountain air and beautiful blue skies. Amidst this setting, the State Veterans Community Living Center at Rifle provides 89 beds of skilled nursing care, including a 18-bed Special Care Unit for residents with Alzheimer's Disease or related conditions. Serving veterans, their spouse and eligible "Gold Star" parents, we are committed to caring for the whole person by addressing the personal and social needs of each resident, as well as their physical and medical requirements. All staff members of Rifle are committed to providing a life of qualify and dignity for every resident. At the Colorado State Veterans Community Living Center at Rifle, our staff is known for showing love and compassion to our residents. Colorado State Veterans Nursing Home at Rifle is a 89-bed, long-term care facility serving the nation’s veterans. It provides skilled nursing services 24/7 and maintains quality of life, safety, and effective care for a diverse and complex population of men and women with varied medical and psychosocial issues.

Standard Assessment
Plans and executes a program of education for all staff with emphasis on nursing service, by scheduling in-services and maintains records to meet Federal, State and VA regulations. Ensures all nursing staff completes continuing education according to regulations. Monitors staff training and continuing education. Develops, reviews, revises and provides education on all nursing competencies and policies and procedures related to nursing.
General Management Administrative
Supervise and direct subordinate staff in their delivery and provision of resident care. Examples are defining job expectations, provide feedback in individual and group supervision, assess individual learning needs and provide staff training, resolve staff conflicts, act as a resource, refer staff to appropriate programs, prioritize and delegate job duties accruing to patient needs and staff qualifications. Participate in job interviews as requested, precept student nurses, assist in orientation of new employees. Complete new employees and current employees annual competency evaluation, ensure compliance with standards of nursing practice, develop performance plans and complete appraisals (PMAP), provide educational memorandums, referrals for guidance of corrective action needs. Completes monthly schedule for assigned shift and updates and revises as needed. Investigates staff on-the- job injuries, completes reports and assists in the development of transitional duty

CNA Instructor
Directs and implements CNA Class. Prepares and submits a course of study including program objectives, student performance objectives and daily lesson plans; pre-screens students, provides related instruction and supervises student training; arranges the CNA certification testing; annually prepares and submits a program budget and is responsible for making requests for materials and supplies not to exceed the amount and types of expenditures set forth in the approved budget; coordinates with Program Supervisor to determine a secure place to keep student’s records for 4 years; submits a report of the total number of hours of training the student received in both theory and clinical and the related skills achieved at the completion or termination of student’s training; ensures instructional quality for the student as assessed by constant mastery and competency based skills; maintain current licensure, level of expertise, and be aware of current changes in discipline by work experience, conferences, continuing education units, course work, journals, etc.

Command Crisis
Collaborate in the provision of patient care with a multi-disciplinary team. Maintain documentation of various systems, which include, but are not limited to skin, falls, weight loss, and others. Examples are: notify appropriate physician of changes in patients mental or physical condition, obtain necessary physician’s orders, facilitate shift reports, act as liaison between shift, head nurse and other clinical management staff, communicate incidents and injuries, etc., attend professional staff meetings, communicate with dietary, operator, lab, hospital, security, housekeeping, etc.

Quality Assurance
Includes 24-hour accountability for delivery of nursing care for each neighborhood. Takes on-call rotation as defined in the facility protocol. Monitor quality improvement at shift and neighborhood level by: taking responsibility for the delivery of patient care according to nursing, nursing home and accreditation standards, compliance with charting by nursing staff (daily and weekly), keep staff informed of changes in nursing home and division policies. Quality improvement reports provided to the Performance Improvement Committee as assigned

Direct Nursing Care

Act as a Charge/Floor Nurse and assume all the duties of that role, including being on the on-call rotation, the treatment and care of a neighborhood unit and directing employees. Responsibility includes addressing attendance and performance/behavior issues and reporting the incident to their designated supervisor.

Continuing Education
Maintain updated skills, knowledge, abilities and competencies necessary to the position through continuing education, clinical supervision, literature review and participating in discipline and team development.

Qualifications

Minimum Qualifications:

Current, valid licensure as a Registered Nurse from the Colorado Board of Nursing or any state
participating in the Enhanced Nurse Licensure Compact (eNLC), in good standing.

AND

Three (3) years of professional experience as a registered nurse experience in an occupational field related to the work assigned to the position.

Substitutions
Appropriate education will substitute for up to two years of the required experience on a year-for-year basis.

Conditions of Employment:
  • Current and Active RN License renewal required.
  • Must be willing to submit to a full background check
  • This position requires travel to conferences and industry specific trainings/meetings.
  • Must possess a drivers' license
  • Must be willing to test for Tuberculosis (TB) test
  • This position has shift work: day, evening and night shifts to include holiday and weekend as needed.
  • This position requires on-call rotation as defined in the facility protocol.
