This position is located at the SC Department of Behavioral Health and Developmental Disabilities (DBHDD) - Office of Mental Health, Columbia Area Mental Health Center (CAMHC), Emergency Services - Housing Coordinator, CAMHC #497.
As the Mental Health Professional, you will serve as the CAMHC Housing Coordinator to ensure patient access to appropriate community-based housing. Services are delivered in the community and patients' homes.
Responsibilities for the Mental Health Professional I:Provide oversight for CAMHC housing programs, including Community Housing, TLC/Housing and Homeless Program Supportive Housing developments, Community Residential Care Facilities (CRCF's), and other supportive housing programs, as applicable.
Coordinate administrative requirements of the agency's housing programs, including payment contracts and patient agreements.
Coordinate with the agency's Transition Programs staff, including Clinical Care Coordinators (CCCs) and Transition Specialists, MCO CCCs, Certified Peer Support Specialists, and Benefits/Entitlement Specialists, Partners with the local HUD Continuum of Care, public housing authority (i.e.), and affordable housing owners and property managers.
Coordinate patient housing referrals and move-ins and maintain wait lists for each Center housing program.
