CO State Veterans Community Living Center at Rifle
The Colorado State Veterans Community Living Center at Rifle is located on Colorado's western slope in the scenic Colorado River valley. Rifle offers a mild climate, with plenty of clean mountain air and beautiful blue skies. Amidst this setting, the State Veterans Community Living Center at Rifle provides 89 beds of skilled nursing care, including an 18-bed Special Care Unit for residents with Alzheimer's Disease or related conditions. Serving veterans, their spouses, and eligible "Gold Star" parents, we are committed to caring for the whole person by addressing the personal and social needs of each resident, as well as their physical and medical requirements. Requirements

Standard Assessment
  • Performs both admission and daily assessments during clinical stay, identifying critical stay, identifying critical incidents, emergencies, acute illnesses and behavior changes with RN oversight.
  • Identifies and implements appropriate nursing interventions, treatment care plan and physician referrals.
  • Documents current, accurate and pertinent information regarding care and treatment provided to individual residents.
  • Indicates specific change in status, medications and physicians orders.
  • Detects abnormality in resident’s health status indicated by symptoms, vital signs and laboratory results and reports information to physician.
  • This position is required to drive a State vehicle to do off-site pre-admission assessments, or take a resident to the doctor of hospital (if not taken by ambulance).
  • Must have an RN co-sign admission assessments.
Regulatory Compliance
  • Directs and implements nursing care to residents by assuring compliance with accepted standards of nursing care, departmental and facility policies and procedures, and VA and State of Colorado regulatory requirements.
  • Directs subordinates to assure these standards are met.
  • Reviews and audits physician orders, MARS, CNA flow sheets, bathing schedules and BM sheets to determine compliance with policies and standards of care as part of a department quality assurance activity.
Medication and Treatment Administration
  • Competently administers medications to residents within scope of practice while observing the five rights of safe medication administration, resident rights, infection control standards, and appropriate nursing policies.
  • Documents all medications given and justification for scheduled medications not given.
  • Appropriately identifies changes in resident’s conditions that need to be assessed by a registered nurse.

Qualifications

Minimum Qualifications:

Current, valid licensure as an LPN from the Colorado Board of Nursing or any state participating in the Enhanced Nurse Licensure Compact (eNLC)


Preferred Qualifications:
  • Ability to follow patient confidentiality standards

  • Excellent interpersonal and communication skills

  • Ability to read and comprehend the English language

  • Ability to effectively communicate fluently in English both verbally and in writing

  • Ability to follow care plans



Conditions of Employment:

FULL BACKGROUND CHECK: This position will have direct contact with vulnerable persons (as defined by C.R.S.27-90-111: "Direct contact" means providing face-to-face care, training, supervision, counseling, consultation, or medication assistance to vulnerable persons, regardless of the level of supervision of the department employee.

"Direct contact" may include positions in which persons have access to or unsupervised time with clients or patients, including but not limited to maintenance personnel, housekeeping staff, kitchen staff, and security personnel.)

  • Full background checks include:
    • CBI & FBI fingerprint check, Colorado Court Database check, Medicare Fraud Database, TRAILS Database check.
    • This position will also provide direct care to vulnerable adults requiring a CAPS check (as defined by C.R.S. 26-3.1-101, position provides services & support care to at-risk/vulnerable adults including case management, clergy, physical or mental health services, safety or welfare services)

  • This position requires State of Colorado residency at the time of application (unless otherwise identified in the posting), and residency within the state throughout the duration of employment in this position.

  • Motor Vehicle Check

  • Must possess a valid, non-restricted Colorado Driver’s License or a non-restricted U.S. Driver's License for positions within 30 miles of the CO state border

  • Out-of-State Driver’s License holders must obtain a valid, non-restricted Colorado Driver’s License within 30 days of employment start date.

  • Must complete and pass TBtestand any other vaccinations as required by the facility.
  • Former State employees who were disciplinarily terminated or resigned in lieu of termination must:

  • Disclose that information on the application.

  • Explain why the prior termination or resignation should not disqualify you from the current position.

  • Provide your employee number from your prior State employment. Absent extraordinary circumstances, prior disciplinary termination or resignation in lieu of termination and failure to provide this information will disqualify the applicant from future State employment with CDHS

    Employment history is calculated on a full-time basis (40/hrs per week). Part-time employment is calculated on a prorated basis to determine qualifications.
