This job involves coordinating with various organizations to help people living outdoors due to homelessness get connected to housing and support services as quickly as possible, while also handling follow-ups on community requests and participating in data tracking efforts.
A good fit would be someone passionate about social services, experienced in working with vulnerable populations, and comfortable building partnerships across government, nonprofits, and neighborhoods.
The role requires flexibility for non-standard hours and a commitment to reducing harm through compassionate, trauma-informed approaches.
Build and maintain collaborative relationships with different Metro departments (with a focus on working closely with the Metro Police’s community outreach and alternative policing efforts), social service agencies (including nonprofit and faith-based organizations), businesses, state government agencies, and neighborhood organizations to advocate, educate, and collaborate with the goal of moving people off the streets and linking them with needed support as quickly as possible; Participate in regular check-ins with Metro precincts, District Council members, and addressing daily HUB requests.
Assist with data entry projects, including promoting HMIS data entry with partner agencies and Coordinated Entry, to help ensure that OHS has a solid overview of what outdoor homelessness looks like in Nashville and to start the coordinated entry process for people living outdoors.Provide direct outreach services and help fill gaps in the community by providing housing navigation to people who are sleeping outside in Davidson County.
Help maintain an accurate contact list of all outreach groups for the Office of Homeless Services and share it with community partners.Co-lead efforts to ensure we have the most accurate Point In Time (PIT) count in our community, and staff other Homelessness Planning Council committees as requested; Staff committees and coordination meetings as assigned.Initiate and support processes that reduce harm to people experiencing unsheltered homelessness and advocate for people living outdoors while encouraging them to access shelter and services.Coordinate extreme weather responses and be available to fill staffing needs if Metro opens overflow facilities including the coordination of Metro’s cold weather response.Assist in staffing phone lines designated to improve the coordination of services and/or outreach to landlords.
Other duties as assigned. Qualifications Bachelor’s degree. *Additional experience may be considered in lieu of degree (1:1 ratio).
