This position is located at the SC Department of Behavioral Health and Developmental Disabilities (DBHDD) - Office of Mental Health, Coastal Empire Community Mental Health Center (CEMHC), 1050 Ribaut Rd., Beaufort, SC 29902.

With the Office of Mental Health’s mission to support the recovery of people with mental illnesses. We provide a complete array of medical and support services for children, adults, and families throughout South Carolina.

As the Engagement Specialist / Counselor II, working under direct supervision of the Director of Emergency & Community-Based Services, you will provide outreach to patients and their families for CECMHC who are not participating in treatment as outlined on their Individual Plan of Care, as well as recommendations for services by their service providers/treatment team.

Responsibilities for this Engagement Specialist:

Guided by the treatment team, help determine if services being provided are meeting the needs of the patient and family and offer recommendations around services needed or recommended.

Contact patients identified on the 45/90 days not seen report and referrals from direct service providers and actively works to engage the patient.

  • Develop specific data collection and reports outcome data to assist with reducing no show and patient cancellation rates, and to utilize in increasing clinical services provided by the Center. Work with supervisors and staff in identifying and reviewing cases to re-engage the patient in services.
  • Maintain all documentation of charts and other records in accordance with State, Federal, Local, ODMH, WCMH, Quality Assurance, Corporate Compliance and CARF Standards.
  • Assist the appointment schedulers as backup assisting with reminder calls, cancellations/reschedules related to staff call-outs as well as staff transitions.
  • Provide coverage for essential support staff duties during critical needs for base coverage in clinics.
  • Attend clinical staffing and treatment team meetings as directed by supervisor and as appropriate.
  • Requirements


    • A bachelor's degree in guidance, counseling, or the social sciences and professional experience in human services or social services programs.


    Qualifications

    • Experience working with children and adults in a mental health setting.

    • Background in customer service.

    • Bilingual abilities in English and Spanish (or another language) are a plus.

    Additional Information

    Post hire, employees must:

    • Be able to complete Managing Agitation in the Psychiatric Setting (MAPS) certification.
    • Be articulate.
    • Be able to coordinate services and understand the community well enough to assist patients with access concerns.
    • Have the ability to establish and maintain satisfactory working relationships with employees and patients.
    • Be motivated, function independently and exercise good judgement.
    • Be able to provide services to all of the Center's clinics.
    • Be able to maintain documentation in patient records and other records in accordance with State, Federal, Local, OMH, CECMHC, Quality Assurance, Corporate Compliance and CARF Standards.
