Correction Specialist 4-State Opioid Response Grant Manager

Tumwater - Project

Salary Range: $7,185 –$9,665 Monthly

The Department of Corrections is seeking a highly motivated and qualified individual to fill the project position of a Corrections Specialist 4 -State Opioid Response Grant Manager (CS4).

Working for the Department of Corrections, Health Services Division, in collaboration with community partners, this position will contribute to staff and community safety by providing oversight to staff delivering individualized case management and continuity of care behavioral and medical services to incarcerated individuals with an opioid use disorder with a focus on access to medications for addiction treatment.

The Health Services case management and continuity of care activities will provide comprehensive health services and release planning specific to the individual identified as meeting current SOR Grant eligibility.

Under the supervision of the Health Services Reentry Administrator and in collaboration with the Substance Abuse Recovery Unit, this position will oversee internal and external collaborative reentry planning and transitional services for program participants statewide.

Continuity of care through case management will start in prison and/or reentry center and continue into the community post-release.

The CS4 position will provide critical continuity of care oversight in meeting the objectives and deliverables of the State Opioid Response Grant as funded.

This position will meet these objectives and goals by supervising the MAT Coordinators (Medication for Addiction Treatment) and navigating the ever-changing landscape of the integrated behavioral, medical care networks and health exchanges.

This position is an integral part of the Health Services Reentry Leadership team, shaping coordinated, comprehensive discharge planning and community linkage for individuals releasing from prison or transferring to partial confinement.

Requirements

Supervise MAT Coordinators, providing specialized healthcare reentry services to individuals with a qualifying substance use disorder releasing from prison or transferring to partial confinement.

  • Plans, organizes and evaluates, assigns and directs work activities of MAT Coordinators and AA3.
  • Ensure Statewide MAT Coordinators are completing tasks following continuity of care grant deliverables, protocol/s, policies.
  • Gather data related to contacts and community follow up
  • Provide annual evaluations and regular performance feedback for MAT Coordinators
  • Provides quality assurance of continuity of care
  • Maintain and Update protocols and documents needed to meet grant deliverables
  • Facilitates Team Education for best practices for continuity of care, specializing in staying up to date supporting individuals with MAT/ Substance Use Disorder needs.
  • Cover Staff caseloads as needed for training, vacancies, other program needs.
  • Communicate and collaborate with facility-based Health Services Reentry Team.
  • Ensure Team documentation meets all DOC and Grant requirements to effectively document work people completed.
  • Attend Multidisciplinary team meetings when necessary to support staff and patients in accessing required care.
  • Identify and report and barrier to access to care and participate in system-level conversations to improve participant access to services and positive patient experience through the program.

Responsible for Grant Management and Program Documentation

  • Maintain comprehensive records specific to grant deliverables
  • Communicate with SARU SOR clinical provider for referral data
  • Track upcoming releases and community follow up
  • Provide HS Data Analyst and SARU Grant Manager with grant deliverables achieved for Grant reporting
  • Maintain a list of participants, release planning activities, and community follow up information
  • Complete SAMSHA reporting as needed for Grant deliverable
  • Complete and maintain accurate, complete and current records/ files.
  • Ensure appropriate and efficient use of resources through work with DOC Grant Staff budget Staff.
  • Ensure drafted budgets meet program needs and that funds are utilized to their full extent.
  • Create, Maintain, and update program forms, documents, processes, etc.
  • Develops, revises, and implements SOR funded program, setting standards and providing accountability and fidelity to the grant.
  • Provides quality assurance to include regular review of program notes/ records to ensure all contract/grant requirements are met.
  • Participates in updating relevant policies as needed.

As statewide subject matter expert on continuity of care for MOUD, build statewide collaboration and ensure continuous engagement with internal and external partners to leverage resources.

  • Participate in statewide workgroups, coalitions, and other community-based activities related to SOR grant collaboration and resource development
  • Engage with internal partners to provide a collaborative approach to continuity of care, including but not limited to Prisons, Reentry, Resentencing, Health Services and Community Corrections Divisions
  • Participate in Teambuilding activities and coordinate regularly with Health Services Reentry Team Leadership.
  • Responsible for ensuring program is in alignment with other HS Reentry programs, with regular efforts to improve process.
  • Regular communication and partnership with MOUD RNs, Addiction Physician, and MOUD Educator
  • Act as MAT Continuity of Care SME for DOC.
  • Assist with education of CCOs, prison staff, and other DOC Staff when possible
  • Understand WACs, RCWs, federal policy on MOUD reentry.
  • Communicate effectively in the evaluation of workload processes.
  • Assist higher level DOC, HCA staff by coordinating all aspects of statewide program; identify solutions to complex problems and issues that arise.
  • Participate in standard work activities and other opportunities to ensure all reentry programs are operating efficiently and in collaboration within DOC.

Other duties assigned

  • Complete training and professional development as required by the Department
  • Participating in educational opportunities related to achieving SOR Grant deliverables
  • Participate in other projects, CQIP, development program metrics, and/or other quality assurance
  • Coverage for HS Reentry Leadership Team as needed and appropriate to role
  • Other duties as assigned.

Qualifications

Required qualifications:

  • Five years of professional social service or case management experience with justice impacted individuals with behavioral health disorders.
  • Two years’ experience supervising staff conducting case management activities.
  • Two years conducting case management with adults.

Preferred/Desired Qualifications:

  • Extensive experience working with people with SUD needs and knowledge of Medications for Addiction Treatment (MAT) and substance use disorder treatment.
  • Ability to educate patients, staff, and partners on MOUD.
  • Skilled with Motivational Interviewing and other evidenced based approaches.
  • Bachelor's Degree with focus in Social Service or related field from an accredited college whose accreditation is recognized by the US Department of Education or the Council for Higher Education Accreditation (CHEA), or a foreign equivalent.

Note: Additional qualifying experience will substitute year for year for up to two years of the desired education. A Master’s Degree will substitute for one year of experience.
