This job involves helping patients at a public health clinic by connecting them to community services, managing referrals for non-medical support, and assisting with applications for programs like AIDS drug assistance.
It's a supportive role focused on patient navigation and follow-up care.

Coordinates referrals with social service agencies in and out of the Ryan White continuum of care.Develops and maintains relationships with community partners to facilitate referrals.Performs and documents initial patient interviews as assigned.Documents all clinic and ADAP eligibility screens and services provided in required databases.Receives and documents referrals from clinical providers.
Develops a plan of care related to referrals before patient leaves the clinic if possible.
Facilitates appointments.Follows up and documents monthly "lost to care" activities.Provides transportation support to eligible patients.
Documents and tracks all transportation assistance tools.Maintains clinic list of all ADAP eligible patients.
Performs initial ADAP eligibility screening and recertification every six months thereafter.Works closely with medical and non-medical case managers in care team to ensure well-coordinated medical care.Attends ADAP trainings annually.Performs all other duties as assigned.
Qualifications The work schedule is: Monday-Friday 8am-5pm TO APPLY, must have a High School Diploma, GED or higher education.TO APPLY, must have One (1) full-time year of working in a social service or healthcare setting.Experience with basic health education preferred.
Experience with HIV/AIDS preferred. Bilingual in English and Spanish (preferred is a plus, but not required). Good organizational and time management skills.
Strong interpersonal and communication skills.
Detail oriented with the ability to follow-up with patients and services providers in a timely manner.Possess outstanding customer service skills, especially with patients.
Basic computer and phone skills.
Ability to focus and complete tasks.If hired, you must provide proof of educational attainment at new hire processing Tarrant County will conduct background checks on new hires that will include a criminal background check related to convictions and deferred adjudications in the past seven years and may include credit reports, motor vehicle records, employment records and educational attainment.
A conviction or deferred adjudication is not an automatic bar to employment. Each case is considered individually.

Exposure to active infectious diseases and body fluids. Possible needle sticks and restraining of children.
