The Department of Health is seeking a caring and hardworking Community Health Nurse to provide education and clinical services in the Southeast District.
By working with partners in the community, you will help us make sure Department of Health programs are available and relevant, as well as investigate threats to public health.

In this position, you will be responsible for planning, implementing, and evaluating public health services and initiatives, as well as providing direct public health services and community liaison activities.
Your work will involve analyzing county level assessments to identify public health needs, linking program resources with community partnership activities, and developing public health initiatives to address identified health priorities.
You will participate on district and local health committees as a representative for the Department of Health to advocate and support the development of public health initiatives.
Your duties will include monitoring community health programs, developing and providing health education programs, and providing case management services to promote patient health and safety.
Additionally, you will provide clinical services and provide consultations to individuals, health professionals, and community partners.
