Job Description

Summary

This job is for an advanced pharmacist who works independently to provide direct patient care, including prescribing medications, in a home-based program for veterans.

Full Job Description

The clinical pharmacy specialist (CPS) functions at the highest level of clinical practice, working independently with prescribing privileges as outlined in the individual's scope of practice, as defined by the individual medical center to directly care for patients.

Available Locations: Poplar Bluff, MO Cape Girardeau, MO West Plains, MO Paragould, AR Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.

Basic Requirements: English Language Proficiency. Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d). Citizenship. Citizen of the United States.

(Noncitizens may be appointed when it is not possible to recruit qualified citizens in accordance with chapter 3, section A, paragraph 3g, this part.) Education Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree.

(NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy.

Graduates of foreign pharmacy degree programs meet the educational requirement if the 2 graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet Based Test (TOEFL iBT).

Licensure Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia.

The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure.

A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16.

May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).

Grade Determinations: GS-13 Preferred Experience: Must have 1 year of experience equivalent to the next lower grade level.

Dispenses medication to hospital inpatients and Community Living Center (CLC) residents through medication management systems. (i.e.

unit dose, automatic replenishment, ward stock, IV admixture, total parenteral nutrition, chemotherapy).

Dispensing is accomplished by using professional knowledge of the principles and practices of selecting and compounding and dispensing pharmaceutical products according to VA policies and procedures to ensure optimum efficiency and compliance with regulations.

May participate in Outpatient dispensing. Dispenses medication, dietary supplements and medical supplies to eligible outpatients at pharmacy window and by mail.

Dispensing is accomplished by using professional knowledge of the principles and practices of selecting and compounding and dispensing pharmaceutical products according to VA policies and procedures to ensure optimum efficiency and compliance with regulations.

May participate in Controlled Substance dispensing. Issuing and accounting for all controlled substances per hospital and VA policies.

Will provide strict security of all controlled substances by reviewing all orders, prescriptions and records.

The CPS for HBPC coordinates, develops, manages and controls the pharmacy practice programs for HBPC.

This includes, but is not limited to medication regimen reviews upon patient admission to HBPC, quarterly medication regimen reviews, post-fall reviews, and hospital discharge reviews, pharmacy student experiential learning, and other applicable items in accordance with scope of practice Provides input to Associate Chief of Pharmacy, Clinical Services and Chief of Pharmacy in establishing and accomplishing the pharmacy's missions and goals.

Assist and be the contact pharmacist for all medication issues related to HBPC.

Medication therapy management: Initiate, modify and monitor pharmaceutical care plans pertaining to medical conditions commonly managed in the primary care setting, to include anticoagulation management as assigned.

Assess the appropriateness and effectiveness of patients' medication regimens by reviewing medical records, other interdisciplinary communications and interviewing/examining the patients as appropriate.

Recommended therapeutic drug regimens take a risk versus benefit and cost versus benefit approach.

Evaluations, recommendations, and management of medication-related issues will be provided in a timely fashion and be based on published treatment guidelines, evidence-based medicine, patient factors, and formulary issues and follow sound medical and pharmaceutical practices Work Schedule: 8:00am- 4:30pm Monday- Friday, Occasional late nights, and weekends.
