This job involves working as a specialized pharmacist in a Veterans Affairs hospital in Shreveport, Louisiana, focusing on managing anticoagulation therapy to help prevent blood clots in patients.

Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.

Pharmacists appointed to direct patient care positions must be proficient in spoken and written English in accordance with chapter 2, section D, paragraph 5a, this part. Education.
(1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree.

(2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT).
Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia.
The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure.
A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16 Grade Determinations: In addition to the basic requirements for employment, the following criteria must be met when determining the grade of candidates.
GS-13, Clinical Pharmacist Specialist Experience. In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level.
Knowledge, Skills and Abilities (KSAs) - In addition to meeting the experience requirements for this grade level, the candidate must fully demonstrate the following KSAs: Ability to communicate orally and in writing to persuade and influence clinical and management decisions.
Expert understanding of regulatory and quality standards for their program area.
Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise.
Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy.
Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.
Preferred Experience: Completion of a PGY1 general pharmacy residency and PGY2 Ambulatory Care/Anticoagulation residency.

Work Environment: Work is performed in an office/clinic setting with minimal risks that requires normal safety precautions; the area is adequately lighted, heated and ventilated.
However, the work environment requires someone with the ability to handle several tasks at once in sometimes stressful situations. Major Duties:
Total Rewards of a Allied Health Professional The Clinical Pharmacy Specialist functions at the highest level of clinical practice, works independently under their individual scope of practice to directly care for patients in the inpatient and outpatient setting.
This includes, but is not limited to, the following: designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments; helping achieve positive patient centric outcomes through direct and indirect interactions with patients, providers and interdisciplinary teams in assigned areas; performing physical assessments; and ordering laboratory and other tests to help determine efficacy and toxicity of medication therapy.
Other duties include but not limited to: A. Administrative Completes quality management data compilation for outpatient and inpatient clinical pharmacy activities as required.
Serves as consultant or as a member of various committees as needed and appropriate.
Participates in treatment teams, committees and other groups needing input regarding the use of anticoagulation therapy or drug policy development as required. B.
Clinical Conducts thorough patient interviews, collects patient specific information to detect and resolve drug related problems and make therapy recommendations.
Develops pharmacotherapy goals, designs treatment regimens and monitoring plans, monitors therapy outcomes and appropriately modifies pharmacotherapy plans. C.
Scope of Practice Works under individual Scope of Practice designated based upon the incumbents' education, training, and experience required to perform the functions of the position.
Serves as an Advance Practice Provider to initiate, modify or discontinue medication therapy. Will participate in a professional practice evaluation program. D.
Education Participates in the training and in-servicing of pharmacists, pharmacy technicians, pharmacy students, pharmacy residents, and other staff on assigned teams.
Serves as preceptor for pharmacy students and pharmacy residents in accordance with established polices. E.
Research Participates in research projects either by acting as a consultant or serving as a co-investigator and participates in the design, conduct and documentation of clinical drug investigations.
Work Schedule: 8:00 am - 4:30 pm, Monday - Friday - to include weekends, holidays and hours outside of designated tour of duty - subject to change based upon the needs of the facility.
