This job involves helping older adults, people with disabilities, and their families by assessing their needs, connecting them to community resources, and providing short-term support to stay safe and independent at home.
You'll spend much of your time visiting clients in the community and offering personalized advice and referrals.

Why it's a Great Opportunity: As a bilingual Case Manager fluent in Samoan familiar with the aging process and case management, you will provide accurate and relevant information to individuals seeking resources including older adults, family caregivers, and adults with disabilities.
You will deliver services through options counseling. You will provide individualized and comprehensive short-term case management services.
You will primarily be helping our customers out in the community visiting with clients each month.
You are on front line of our efforts dedicated to connecting older adults and adults with disabilities with services to help them remain safe and independent at home. Here, our assistance has a direct impact on our client's quality of life.

How to be Successful in this Role: We are looking for someone with case management experience and knowledge of person-centered care.
Someone who can assess client needs and make individualized recommendations in partnership with families and other support individuals.
You will be diligent, and detail oriented with documentation and can write in a clear and concise manner.
You will communicate across various social, cultural, educational and economic backgrounds so you should be able to adapt quickly based upon audience.
Your Future in this Role: Throughout the onboarding process, you will meet with the Managers, Supervisors, and teams, regularly to discuss expectations, set goals, identify priorities, and gain familiarity with current policies, procedures, and strategic measures.
