Job Description

Summary

This job involves helping older adults, people with disabilities, and their caregivers by providing information on resources, assessing their needs, and offering short-term guidance on services like care transitions and Medicaid support.

It requires working independently with strong customer service skills, mainly in Aberdeen, Washington, with some travel to nearby areas.

Key Requirements

  • Bachelor’s Degree in behavioral, health sciences, or related field with two years’ experience providing direct human services, or Master’s Degree with one year experience, or equivalent combination
  • Experience conducting eligibility screenings and needs assessments for programs and services
  • Ability to deliver options counseling and short-term case management for older adults, caregivers, and individuals with disabilities
  • Exemplary customer service skills and ability to operate independently with minimal supervision
  • Current Washington Driver’s License and access to an insured vehicle for required travel
  • Knowledge of community-based programs including Care Transitions, Health Related Social Needs (HRSN), Medicaid access, and WA Cares

Full Job Description

AGING AND DISABILITY RESOURCE CENTER (ADRC) SPECIALIST wanted for Olympic Area Agency on Aging (O3A) based in Aberdeen, WA with some required travel to Raymond, WA.

Requirements

The ADRC Specialist Provides accurate and relevant information to individuals seeking resources including older adults, family caregivers, adults with disabilities and Medicaid/WA Cares beneficiaries. Conducts program and service eligibility screening and needs assessments. Delivers services and options counseling under targeted community-based programs, including: Care Transitions, Health Related Social Needs (HRSN), Medicaid access, and WA Cares. The ADRC Specialist provides individualized and comprehensive short-term case management services. Operates with a significant level of independence, requiring minimal supervision. Requires exemplary customer service skills.

Qualifications

Requires a Bachelor’s Degree in behavioral, health sciences, or related field with two years’ experience providing direct human services; or MA in behavioral, health sciences or related field with one year experience providing direct human service; or equivalent combination of education and experience.


Requires current WDL & insured vehicle.


Additional Information

40 hrs./wk. Non-Exempt.


$59,646 to $69,375 annually.
